Tracy and Michael are concerned about how to break it to Alice that she won’t get Rafe’s heart after all. Tracy promises to find Alice a better heart but wants her to keep quiet about about what she heard. Silas tells Nina that he heard her waking up during the night shouting out Rafe’s name. Silas enters Rafe’s room and Sam tells him his heart can’t be used for the transplant because of his drug use. Nikolas promises to take Spencer to see Alice if he behaves himself with the new counselor. Britt and Spencer continue to conspire after she shows up. Joss hears them talking and demands to know what they are talking about. Elizabeth sees Britt and Nikolas talking in the park. Kiki and Morgan support each other about Alice and Rafe. Molly remembers somebody put Rafe up to driving the car off the road but forgets again after she wakes up. She then learns that Rafe is brain dead. Michael returns to Alice’s room and hears Tracy asking Alice if they have a deal. Alice tells him that they just agreed to both do whatever it takes to find a new heart. Rafe's family and friends gather as his life support system is turned off. Nina sees Rafe comforting Sam.
